Next, the nurse takes the patient to the examination room.

When an exam room is available, the nurse goes to the front desk and takes the clipboard with the patient's medical information. The receptionist has already added to the clipboard the shadow chart consisting of the last progress note, demographic form, medical history form, fee slip, all lab reports that arrived since the last visit, and any other documents that will be needed for the patients visit.

The nurse then goes to the waiting room, gets the patient and takes them back to the exam room.

The nurse begins the evaluation process, gathering as much information from the patient as possible prior to the doctor examining and treating the patient.

If pictures were taken at the previous visit, they are pulled up onto the computer screen for the doctor when he/she examines the patient.
