A welcome packet is sent to new patients.
Every new patient is sent a welcome pack before their scheduled appointment. The welcome pack includes a medical history form, a patient demographic form, and a HIPAA form. The patient is asked to complete these forms and to bring the completed forms to their appointment.  back
